1) Zero Tolerance Cheating Form - Mathematics Use this to report a cheating incident. Download to your computer, fill out and email, as an attachment, to the chair. The chair will forward on to the VP of Student Services.

2) Cheating Blurb Include this in your syllabi. Notice you must state in your syllabus the amount of points you will deduct for a conduct code violation. If you do not use total points, define how the violation will affect the student’s grade.
