All forms must have the signatures of the department chair, articulation officer, and librarian before being sent to the Curriculum Committee if they are required on the form.

1. Complete/detailed course content for each module needed.
2. Changes in these course attributes require district discipline agreement when course are taught at more than one college in the district.
3. If validation has been completed previously, and the change is to recommended, no requisite and validation forms are necessary.
4. Existing course outline from other college which was used as a model must be included.
5. State form " Application For approval of Credit Course".
6. Certificate of Achievement form.
7. State and District forms for new certificate or program approval.
8. State form "Non-Substantial Changes to Approved Program (or Reactivation of Inactive Program)
